To demonstrate local impact, let's take a look at just three providers - Hope, Inc., Denton County MHMR and Health Services of North Texas -&nbsp;that operate transitional and permanent supportive housing programs.</p>
<p>In 2012, these providers housed nearly 100 formerly homeless persons, half of whom were homeless families. Based on the analysis, most residents arrived by referral from other nearby providers, including soup kitchens and shelters. By allowing clients to share relevant information and enabling providers to coordinate resources, THN and our partners can reduce overall homelessness and ultimately promote higher quality of care for those in need.</p>
<p>I'd like to thank all those who participated in <a href="http://www.thn.org/hmis-implementation/">Texas Homeless Management Information System</a> training last week, including Chrisitan Community Action, Our Daily Bread and The Salvation Army. THN is proud to work with local providers and stakeholders to prevent and end homelessness in Denton County.</p>]]></content></entry><entry><title>Community Empowerment in Brenham</title><category term="Brenham"/><category term="HMIS"/><id>http://www.thn.org/blog/2012/10/18/community-empowerment-in-brenham.html</id><link rel="alternate" type="text/html" href="http://www.thn.org/blog/2012/10/18/community-empowerment-in-brenham.html"/><author><name>Daniel Gore</name></author><published>2012-10-18T18:14:50Z</published><updated>2012-10-18T18:14:50Z</updated><content type="html" xml:lang="en-US"><![CDATA[<p>Earlier this month, <a href="http://www.faithmission.us/" target="_blank">Faith Mission</a> in Brenham officially adopted the&nbsp;<span>Texas Homeless Management Information System (HMIS), a case management resource utilized by over 60 providers across the state. HMIS enables providers like Faith Mission to better coordinate emergency assistance and care, while measuring their clients' progress over time toward self-sufficiency and stable housing.</span></p>

Their ongoing efforts include the provision of shelter, meals, medicine, clothing, household supplies, rent and utility assistance, as well as various counseling services.</p>
